Community Support services assist individuals with certain mental health diagnoses to develop a plan and obtain the skills and resources that allow them to live a more healthy, stable, and fulfilling life.

Extending Care to the Community

At Mid-Plains Center, our Community Support program is dedicated to assisting adults with serious and persistent mental illness (SPMI) or substance use by connecting them with essential resources, referrals, and recovery skills. Our team works closely with the client and their other care providers to develop personalized treatment plans aimed at promoting independence and improving quality of life. Understanding the unique challenges faced by individuals with SPMI and addiction, our Community Support team walks alongside our clients each step of the way as they progress in their recovery journey.

Our services extend beyond the clinic, as we can provide our services in the clinic or in the home and community to ensure the highest level of coordination and continuity of care. By offering support in familiar environments, we can more effectively address the diverse needs of our clients, fostering a sense of stability and empowerment. Whether it’s helping with daily living or recovery skills, connecting to community resources, or building emotional strengths, our Community Support team is committed to enhancing the well-being of those we serve.

If you have Medicaid and are eligible for Community Support, there is no cost to receiving this service.  Call our clinic if you would like to learn more and see whether you may qualify for this service!
