Extending Care to the Community
At Mid-Plains Center, our Community Support program is dedicated to assisting adults with serious and persistent mental illness (SPMI) or substance use by connecting them with essential resources, referrals, and recovery skills. Our team works closely with the client and their other care providers to develop personalized treatment plans aimed at promoting independence and improving quality of life. Understanding the unique challenges faced by individuals with SPMI and addiction, our Community Support team walks alongside our clients each step of the way as they progress in their recovery journey.
Our services extend beyond the clinic, as we can provide our services in the clinic or in the home and community to ensure the highest level of coordination and continuity of care. By offering support in familiar environments, we can more effectively address the diverse needs of our clients, fostering a sense of stability and empowerment. Whether it’s helping with daily living or recovery skills, connecting to community resources, or building emotional strengths, our Community Support team is committed to enhancing the well-being of those we serve.

Currently, Medicaid is the only payer we can access for Community Support. If you qualify and are approved, there is never any cost to you.
Our staff will meet with you to collecting basic information. Only certain diagnoses, including Serious and Persistent Mental Illness (SPMI) and substance use, are able to be authorized. Our staff are happy to help you understand if you are likely to be eligible.
Yes! We now have a handicap accessible entry, restrooms, and staff are ready to assist in navigating any other parts of the building you may need to access. Additionally, Community Support can take place in your home or in the community, and our staff can transport and accompany you to appointments that are part of your treatment plan.
